SPECIAL OFFER TO ARKANSAS RESIDENTS:
A deferment of $300.00 will be provided for up to 35 Arkansas residents and owners and staff of Arkansas licensed long-term care facilities who register prior to May 15th. To qualify for a possible deferment, eligible individuals and entities must pre-register by sending a fax to Carol Shockley, Director, OLTC, at 501-682-1197. The fax must include the name or names of all registrants, position titles and facility affiliation if applicable, a contact person, and a means of contact. In the event that more than 35 eligible individuals register prior to May 15th, the Office of Long Term Care reserves the right to award deferment in an effort to promote the widest diversity of participants. Once you receive
notification from OLTC, you must REGISTER ON THE EDEN ALTERNATIVE WEBSITE AT WWW.EDENALT.ORG. You will receive a passcode to use in registering. Please do not send registration fees to OLTC. Sending fees to OLTC will delay or possibly prevent registration.
