I would like to know if anyone has developed nursing admission assessment form keeping within individualized care? I have found a few, but none seem to be comprehensive. I'm reaching out to see if their is something that is available that touches on both individualized care and the clinical

Hello Nelia!

We have just formed a core group to look at the way the admission process happens. We have members from every department and elders who are helping with the process. It's amazing the amount of paperwork that it takes to become one of our elders. It can become overwhelming to families and tiring for the elder.

We are in the beginning process of creating an elder notebook that would assist in the admition process. It would have information on Klein history, their caregivers, meals, activities, daily life, and care plans just to name a few. We are working on a welcome basket where every department would introduce themselves to the family and elder and put a gift in their basket. (i.e. social services may give stationary and stamps, rehab may give them a stress ball, fun stuff like that.)

We are also going trhough our paperwork to make sure that five different people aren't all asking them the same thing over and over and over....
