This unique facility and its programs restore hope to a wide range of individuals - from those who need help as they recover from an injury or illness to those with a permanent disability or loss of function who need long-term care. We are one of three homes in the State of Colorado with an in-house child day care center. Our Bent County HealthCare Center and Prairie View Assisted Living Center team believes in an interdisciplinary approach that focuses on the whole person: physical, emotional, spiritual, and social. Our team of professionals, staff, and consultants provide a comprehensive range of services for elders and neighbors in the skilled nursing or assisted living facilities. We offer Medicare certified skilled services, long-term nursing home care, special care unit, rehab services, restorative programs, Hospice care, and respite care/adult day care. We have intergenerational activities with our elders, neighbors, and children. We believe in the principle "that human growth must never be separated from human life".
