Horizon Health and Subacute Center in Fresno, CA renewed their Eden Registry status on May 28, 2010. This is home to 180 Elders with diverse needs including long-term care, sub-acute care and short term rehabilitation services. Regardless of the needs for support services, all Elders are valued for their individuality within this home. The new Memory Garden they are creating together is a great reflection of that as many people are involved in the development and completion of the garden, including writing their names on pebbles that are incorporated into the design. The home has a mobile garden that the Elders and families help take care of. In the fall, they coordinate with local farmers to create a Farmers Market in the home and the Elders stop in to taste and take home produce of their choosing. When an Elder is experiencing end-of-life, the home gives the Elder a pocket angel as they explain the support services that are available to guide them through this life passage. Breakfast is a big social event in the home. Food of the Elders' choosing is available when they arrive. They sit with their neighbors to visit and catch up on current events. Other meals are served in courses, similar to what you would have in a restaurant. When an Elder moves into the home, they try to learn as much as they can about them quickly to help them become well-known in their new community. Each Elder has a shadow box outside of their room with pictures and other items to tell a bit of their story. In their room you will find a list of "The Five Most Important Things I Want You to Know about Me." It is a great list to help guide the fulfillment of simple pleasures, identify common interests and spark conversation. Becoming well-known is important in this home.
