Our Hillcrest Firethorn location, 8601 Firethorn Ln, Lincoln, NE, is actively seeking a knowledgeable, experienced MDS/RAI Coordinator candidate. This will be a fast paced, dynamic facility whose work environment resembles that of a ‘Med-Surge’ unit. This service line is part of the area’s largest continuum provider of health care services to aging adults. Our continuum of services includes IL, AL, Memory Support, LTC, Post-Acute Care, outpatient Physical Therapy, Home Care and Hospice services to name a few. The MDS / RAI Coordinator plays an integral role in helping our guests reach their therapy and transition goals.

Responsible for patient’s admission and the coordination of care through the transition process.
Responsible for compliance with Federal, State and Medicare regulations and guidelines
Fiscally responsible for the effective management of each post-acute patient and the appropriate completion of the RAI/MDS process
Provides billing dept. with all the HIPPS codes and ensures all patients have appropriate diagnosis codes for billing facilitates the completion of the Omnibus Reconciliation Act (OBRA) and Prospective Payment System (PPS) assessments


At least 2 years’ experience in an MDS/RAI position in a skilled nursing facility or Post-Acute environment
At least two years’ experience in performing nursing duties within an acute or post-acute facility
Experience in the completion of the RAI process and implementation of Medicare regulations and guidelines
Must be licensed as a Nurse (RN or LPN) in the state of Nebraska or Compact State
